description

The Universal Care Plan (UCP) Programme, hosted by NHS South West London manages the implementation, maintenance and digital development of a digital personalised care and support plan application on behalf of all London Integrated Care Boards.

The UCP's digital platform is provided by Better, and uses OpenEHR technology, which enables clinicians across London to co-create and share personalised care plans across clinical and organisational boundaries: Universal Care Plan - NHS South West London ICB (onelondon.online).

The UCP system integrates with various Electronic Patient Record (EPR) systems which includes EMIS, TPP SystmOne, Adastra, Cleric, the London Care Record, and the National Record Locator, allowing clinicians to easily access care plans through their native system. For those organisations that do not have an EPR, users can access care plans through the UCP Web Portal via their browser.

As of January 2024, patients can view their care plans through the NHS App. The UCP continues to deliver its digital roadmap which will include integration with Cerner Millennium, Epic and Rio
