Sheffield City Council are looking to improve outcomes for adults with multiple disadvantages by using a joined-up and person-centred approach through the Changing Futures Programme. These adults could be experiencing a combination of homelessness, substance misuse, mental health issues, domestic abuse, and contact with the criminal justice system. Part of Sheffield City Council's delivery approach includes a core team of case workers to engage and work with service users intensively and the provision of bespoke interventions to domestic abuse perpetrators and victims with complex needs, including where there are allegations of mutual abuse. Local data intelligence suggests there is a cohort of around 200 people who are already known to services in Sheffield who would be eligible to be part of the Changing Futures Programme.
