Reviewing of patients on anticoagulation medication and providing qualified and expert support to switch to alternative forms of medication that avoids patients needing to physically attend blood test clinics.

Additional information: The CCG reserves rights to novate this contract during its term, to a future lead provider of local community services, where the CCG may assess it to be appropriate to achieve such integration.
