People Keeping Well is part of Better Care Fund and supports people who are mainly in need of 'early help' (on the cusp / edge of care) - primary / secondary prevention

[NB this is different to many other SPx schemes who work with people at risk of immediate non-elective hospital admission - tertiary prevention]

Social Prescribing

The PKW approach is focussed on early help and community based prevention activity to improve people's health and wellbeing. It is based on the principles and techniques that are known as social prescribing.

It is well evidenced that social factors are a large contributor to health and wellbeing - many people experience declining health and wellbeing due to social issues (e.g. loneliness, bereavement, unemployment, money / housing problems, low to moderate mental health, managing one (or more) long term condition.

By ensuring people are connected to and feel part of their local community we can help them stay independent and well for longer and increase their quality of life. By resolving social issues and connecting people to 'what matters to me', through PKW, we hope to empower people and develop skills.
