Care Navigators, Whole Systems Integrated Care

As part of Harrow Clinical Commissioning Group's (CCG) plan for developing the delivery of a Whole Systems Integrated Care (WSIC) model for Harrow’s older adult residents, we are looking to commission the delivery of the Care Navigator aspect of the system.

Care Navigators are currently in post and will transfer to the new provider under TUPE regulations. The role of the Care Navigator is to provide navigation support to patients and local health economy colleagues across primary care GP practices, six peer groups and three virtual wards that support patients with long term conditions to stay at home to be treated. There is one Care Navigator per peer group.

Interested parties are now invited to express their interest and download the Invitation to Tender (ITT) documentation.

The CCG anticipates awarding a contract 1 year. The expected service commencement date is 2nd May 2016. The total value is estimated at £250,000 for the intended 1 year duration of this service (whilst the CCG reserves the right to extend this contract for up to 3 years).
