description
- The ICES in Sheffield will provide customers with the equipment and some small adaptations that they need to stay independent and to live at home. 1.1.2. The Service covers all ages - children, young people and adults - living in Sheffield or living outside of Sheffield but registered with a Sheffield GP or supported by a Sheffield-based Prescriber, though many aspects of the Service also apply to their families and carers. If in doubt as to a person's eligibility, the Provider must contact the Commissioner for a discussion/advice. 1.1.3. Equipment will be provided on loan to people living in the community. Living in the community is defined as where the customer is staying in Sheffield, such as their own home, with family on short term basis, a residential care home or intermediate care facility. 1.1.4. The Provider will source, procure, deliver, install, repair, service, recover, clean and refurbish equipment and some minor adaptations (such as grab rails) for children, young people and adults living in the community in Sheffield. 1.1.5. The Service will provide information and advice to customers about equipment use and about other services in Sheffield. 1.1.6. The Service will appropriately enable customers to self-fund the purchase of equipment. 1.1.7. The Service will facilitate the purchasing of Special (i.e. not Standard Stock) equipment by the Commissioners.
