NHS Sheffield Clinical Commissioning Group (CCG) and Sheffield City Council (SCC) have been working in partnership with Sheffield Teaching Hospitals NHSFT (STH) to:
| • | Help reduce emergency admissions to hospital |
| • | Support timely hospital discharge, and; |
| • | Wherever possible, discharge people to their own home or normal place of residence. |
Somewhere Else to Assess is a model that supports individuals who are medically fit to be discharged from hospital, but would benefit from a period of assessment in a care home setting while their long-term care and support needs are identified.
This model is a key element of the Health and Social Care vision to:
| • | Develop services that keep people well at home |
| • | Reduce unnecessary hospital admissions |
| • | Reduce the time people have to stay in hospital when they are well enough to leave and; |
| • | Only utilise hospital care when clinically necessary and only for as much time as is needed to resolve the acute need. |
Assessment beds allow 'future needs conversations' to take place outside of an acute setting, reducing the person's risk of infection and/or decompensation in hospital and ensuring discussions about the future don't take place at a point of crisis when their needs are likely to be higher. The aim is always for people to have an opportunity to return home.
