Award of Community Link Worker services Framework for communities in East Lothian in the current context of social distancing and other restrictions arising from the COVID-19 pandemic. The Community Link Worker services will use a social prescribing approach to tackle health inequalities. The aim of the Community Link Worker services will be to work with individuals to promote health and well-being through increasing their knowledge and use of community resources, and developing community support to help them reach their identified goals. Community Link Worker services will provide targeted support to individuals (aged 18 and over) whose health and wellbeing is negatively impacted by a range of issues, including social isolation and socioeconomic circumstances. Providers will receive referrals from GP Practices and ELHSCP Primary Care sources.
