Greater Glasgow and Clyde Health and Social Care Partnerships (HSCPs) wish to commission a specialist support service for people affected by Huntington’s Disease (HD).

The HD Specialist service is expected to provide a person-centred, flexible, family-orientated model of care which provides support throughout the many years which people may live with the condition. Key roles of the HD Specialists include holistic assessment, care planning and coordination, assisting individuals to navigate the health and social care system, emotional support and provision of information and advice about HD to patients and those involved in caring for them
