The Reablement Care Service shall provide short-term, outcome-focused support to eligible Enfield residents who require assistance to regain, maintain or improve their independence following discharge from hospital.
The externally commissioned support will focus on hospital discharge pathways only and will be underpinned by a strong reablement approach. The service will provide time-limited, person-centred support in residents’ own homes, with the primary aim of enabling individuals to regain confidence, relearn or restore daily living skills, and continue to live as independently as possible within their own home.
Suppliers will be expected to take an enabling, outcome-focused approach, doing with rather than doing for individuals wherever appropriate. Support should help people rebuild practical skills, establish safe routines, improve mobility and confidence, manage everyday tasks, and reduce or avoid the need for ongoing domiciliary care wherever this can be achieved safely.
The Council is seeking market insight on the service delivery models, solutions and areas of emerging best practice that could support the Council’s current requirements and inform improvements to future service delivery.
Information provided through this exercise will be used for information-gathering purposes only. It may help the Council understand solutions that are available in the market and may inform and shape any future requirements, reflecting common approaches across the marketplace where appropriate.
As outlined above, the intention of the Service is to ensure the provision of high-quality, accessible support for eligible residents requiring reablement support following hospital discharge, promoting independence, safety and wellbeing
Due to the need for a borough-wide service, the Council is currently considering dividing the future service into four (4) Lots, potentially split by four geographical areas: North West, South West, North East and South East of the borough (please refer to the Indicative Service Specification, Appendix 2 and the accompanying Introduction plus Appendix 2.A and Appendix 2.B for further information). Under this model, it is envisaged that four (4) Suppliers would deliver the Council’s next generation Reablement Care Service within a defined geographical area. The Council would welcome organisations’ views and feedback on this potential service delivery model through the SMT Questionnaire.
An indicative Service Specification, Appendix 2, and its accompanying documents (Introduction, Appendix 2.A and Appendix 2.B) are provided as part of the Preliminary Market Engagement documentation.
