BNSSG ICB (the Commissioner) is seeking to secure a suitably qualified provider with the capability and capacity to deliver a community-based pre-diabetes programme in South Bristol.

The service will provide a preventative intervention for adults aged 18-50 diagnosed with pre-diabetes, aimed at reducing progression to Type 2 diabetes, improving health outcomes, and addressing health inequalities. The service will operate as a person-centred, community-based model integrated with primary care and will complement existing services, including the National Diabetes Prevention Programme (NDPP).

The provider will:

Undertake a 1:1 assessment within six weeks of diagnosis and develop a personalised support plan.

Deliver education and self-management support to enable sustainable lifestyle change.

Provide follow-up at six months to review progress and outcomes.

Support individuals to access local services, acting as a link between primary care, community provision, and voluntary sector support.

The service will focus on engaging underserved populations and will be delivered across a range of community settings, including evenings and weekends, to maximise accessibility.

The contract will be awarded for an initial three-year term, with the option to extend for up to a further two years.

The service will commence on 18 January 2027.

NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board (BNSSG ICB) and NHS Gloucestershire Integrated Care Board are working together in a formal "clustering" arrangement to prepare for their merger into a single new organisation (name to be confirmed) effective from 1 April 2027.

This procurement was carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners. SCW is currently subject to restructuring and, as a result, management of the procurement may transfer to a successor organisation during the procurement process. Bidders will be notified of any such change and of any related administrative arrangements.
