Multi-agency, multi-disciplinary team for adults with treatment resistant psychosis requiring rehabilitation and reablement in the community.
This group of specialist professionals will form a core team focusing on care and treatment of a cohort of people with SMI & psychosis requiring rehabilitation. The team members will be expected to enable and promote access to wider system support whilst retaining responsibility for their clients' care.
