description
Royal Borough of Kensington and Chelsea (RBKC) will be recommissioning Day Opportunities – For Adults with Severe and Enduring Mental Illness Service.
The aim of this service is to:
•Ensuring the existing service users of day services are offered seamless support at the start of the contracts;
•The phased move from a model that primarily funds services through block payments to one that funds people through Personal Budgets and Direct Payments with a clearer focus on inclusive meaningful activities based in the community;
•How the services can be reconfigured to support the further integration of the health and care system in line with the NHS Long Term Plan that is underway, including exploring new models of cooperation and partnership working that will enable more seamless support across the whole care pathway;
•Relocating some building based activities to a new accessible community hub in the North of the borough;
•Using co-production to redesign new service elements with people with lived experience of SMI;
•Developing the strength and asset based approach that will enable people to utilise their own resources as part of the solution to managing their health and wellbeing better;
•Identifying and improving outcomes delivery to give people more choice and control over their support; encompass the user’s perspective in measuring quality and performance; improve efficiency and value for money; deliver improved social value and return on investment; and reduce upstream demand on the health and care system.
