description
NHS South West London CCG on behalf of all London CCGs is lead commissioner for a Dynamic Integrated (Urgent) Care Plan (UCP). The UCP is predominantly a software service that enables the sharing with urgent care services (111, London Ambulance Service, Emergency Departments in Acute Trusts) of treatment escalation plans, crisis care plans, end of life care plans, patient specific protocols and personalised care plans that have been co-developed and documented by different health and care disciplines.
There were 50,588 Londoners, 0.5% of the London population, who had a co-created urgent care plan documented on the existing software service in December 2020. Of these, 4324 were viewed 9704 times by urgent care services in that month.
