description
NHS South West London Integrated Care Board (ICB) is currently developing their service specification and contracting process for the The Provision of Live Love Later Life services in Croydon.
The framework for Enhanced Health in Care Homes, (EHCH) supports care home residents to access coordinated and integrated enhanced primary care through better collaboration between health and social care, the VCSE sector and care home partners. The Healthier Lifestyle project is an ongoing project in Croydon and has demonstrated success in producing benefits for service users. The Live Love Later Life (LLLL) is a new project linked to the Healthy Lifestyle project and supports the care homes specifically. LLLL Age UK Croydon is developing collaborative and integrated working with both external partners in the Borough and the various services within Age UK Croydon. It must be noted that LLLL is providing low level service in these areas and will benefit from collaborating with other providers to support care home residents in a number of projects.
