The Provision of a Community Warfarin Anti-coagulation Therapy Initiation and Monitoring Service (the “Service”) has been contracted for a period of 24 months, commencing on the 1st August 2015, with the option to extend for a further period of 12 months. The contract (NHS Standard Contract as mandated by NHS England) will be procured using the Any Qualified Provider Process (AQP).
