Camden Clinical Commissioning Group Community MSK Services

NHS Camden CCG is keen to test the potential provider market for Integrated Community Musculoskeletal (MSK) Service.

The integrated community element of the MSK service will be either delivered via a lead provider model or form part of an alliance model with acute providers and will deliver an integrated pathway end to end for the whole system.

The alliance contractual model consists of a set of providers that enters into a single arrangement with NHS Camden CCG to deliver MSK services. All providers within the alliance share risk and responsibility for meeting the agreed outcomes and will be equal partners.

The lead provider model is a single organisation or consortium which manages and delivers agreed services, some of which maybe sub-contracted to other providers to deliver agreed services.

The integrated MSK service model benefits include patient centred services with MSK treatment in care close to home or work, improved patient experience, supported patient outcomes with measurable improvements, improvements in pain score reductions, and activities of daily living and confidence in in self-management of patient’s condition, improved access to treatment and MSK services, single point of access to MSK services, improvement in clinical outcomes for MSK patients and a reduction in multiple referrals and unnecessary repeated investigations.

The integrated service will reduce the need for multiple visits and will ensure services are patient centred with patients being treated in the right place at the right time by the right person. The community element of the integrated MSK service will provide community outpatient physiotherapy, orthopaedic, pain, rheumatology services clinically appropriate to be delivered in a community setting, psychotherapy services, residential inpatient rehabilitation services, domiciliary services for Camden residents that are housebound.

Providers delivering the community element of the integrated service will need to work with the other health services delivering MSK services and ensure that pathways are joined up and patients receive a seamless service.

Providers will either manage or contribute to clinical triage, assessment, treatment and therapeutic services to the registered GP population of Camden ensuring prompt onward referral to appropriate service where required.

The contract for all providers within the alliance/or lead provider will be paid on a partial block contract, percentage to be agreed with the remainder paid on a set of outcome based with defined KPI outcomes measures

The contract will run for 3 years with a possibility of two year extension.

Please note that this is a market testing exercise to inform the CCGs’ decision making process. This does not commit the CCG to a carry out a procurement process.
