This is an AQP advert published on behalf of the 5 Nottinghamshire County CCGs to provide Continuing Health Care Services for Nottinghamshire County CCG’s (excluding Nottingham City CCG) registered patients ONLY for a term of 9 months from 1st October 2015 - 31th July 2016 to align with the new AQP opportunity commencing 3rd August 2016. This is a repeat window of opportunity that allows for additional Nottinghamshire Care Home registered providers ONLY to join the existing Any Qualified Provider list (the AQP service providers’ list has been running across the East Midlands since 2011) and provides NHS Continuing Health Care (CHC) services. Please Note: The providers who are already on the existing AQP list do not need to re-apply. The AQP process focuses on new, fully-funded packages of care for patients. This includes patients with Physical Disabilities, Learning Disabilities, Mental Health (including Dementia), Brain Injury and End of Life Care, for specified age groups 'under 65' & '65 and over'. The Nottinghamshire County CCG Commissioners (excluding Nottingham City CCG) would like to encourage providers to apply to supply to the Nottinghamshire County CCGs (excluding Nottingham City CCG) only. Any provider will be eligible to join the Qualified Provider list as long as they can demonstrate competency against the specified criteria. As a Qualified Provider there are no guaranteed levels of activity and payment is made based on actual provision. All providers who wish to apply must do so by completing the Qualification Questionnaire and Technical Questionnaire which can be accessed through BravoSolution. Providers should read the Qualification Information document and all other supporting documents, available for download on Bravo. An electronic tendering tool is being used for this procurement. Interested parties should access the portal: https://ardengemcsu.bravosolution.co.uk/web/login.html.

Additional information: Suppliers using the portal for the first time must register to receive a password for their initial login. To access the ITT: a) Log into the portal and click the “View Current Opportunities and Notices” link; b) Click on the Project Title: AQP Nottinghamshire CCGs Continuing Health Care ; c) Click the “Log in or Register to Participate” button on the left hand side. Once you register your “Expression of Interest” - this will move the ITT into your “My ITTs” page (a secure area reserved for your projects only); d) Click on the ITT code (ITT_ 267) and the “Attachments”, “Qualification Envelope - Questionnaire”, “Technical Envelope - Questionnaire” and the “Messaging Service” can be accessed. If support is required to access the system please call the Bravo Helpdesk on 0800 368 4850. For all other enquiries, interested parties must use the messaging service on the e-tendering Portal. Parties wishing to submit a bid for the Contract and to gain access to the Invitation to Tender documentation, are asked to do this in accordance with the instructions set out above (It is important to note that if you do not register on BRAVO, you will not be able to access the supporting documents/information). Interested parties should note that the deadline for submission of Applications is 12 Noon, 24th August 2015.
