NHS SBS on behalf of NHS Heywood, Middleton and Rochdale Clinical Commissioning Group (the CCG), working with RBC would like to invite expressions of interest to attend a Market Engagement Event on Friday, 14th August 2015, to provide an Integrated Neighbourhood Teams (INTs) Service which will inform the market of the intentions to provide the INTs Service. It is proposing a new service model for Integrated Community Nursing, Therapies and Specialist Nurses to support INTs that will improve the delivery of care for people in the community by integrating systems to enable staff to work more effectively across organisational boundaries.
The INTs Service aims to improve integration between Primary Care, Community Adult Health Services and Adult Social Care to ensure a single integrated pathway of care providers and the voluntary sector to ensure timely, safe transfer from hospital to home to address the rising tide of unnecessary ‘default’ admissions in all our local hospitals.
The INTs Service will accept referrals and use risk stratification i.e. identifying the patients within each GP practice who have, or are at risk of developing, the most acute healthcare needs. Patients will see their care managed through multidisciplinary teams which will include the 3rd sector and Mental Health services across the Borough. It will also provide preventative and lifestyle interventions.
The Provider will understand the health needs of this population and through a process of assessment, planning and support, will improve the health and wellbeing outcomes and will be accountable for ensuring that outcomes improve and inequalities in health are reduced. The Provider will deliver a highly visible and accessible service that takes advantage of developments in the digital world.
The Provider will need to work across all local health and wellbeing services in an integrated way to improve outcomes and will work in close partnership with other Providers in Rochdale, including other NHS organisations, RBC, and the voluntary sector. This integrated approach seeks to ensure that the Provider thinks in terms of delivering care pathways and outcomes rather than services labelled by the name of the profession providing them, and they will have the freedom to configure cross cutting teams to achieve joint outcomes.
Additional information: The CCG are therefore organising a Market Engagement Event and would like to invite interested parties and offer the opportunity to:
- Share a vision and proposal on the way forward
- Gather views and comments from a range of providers and interested parties
- Understand the commissioning intentions and links to relevant RBC Public health services and procurement processes
The CCG will present its outline service model and invite provider feedback to help shape the CCG requirements at the event. The Market Engagement Event will also provide opportunities to:-
- Understand the current provision
- Understand Commissioners future objectives
- Better understand the forthcoming procurement process
- Meet and network with peers and/or potential consortium partners
The CCG would like to invite interested providers to attend the Market Engagement Event on Friday, 14th August 2015, held at Middleton Masonic (The Pavilion), Manchester Old Road, Middleton, Manchester, M24 4DY. Providers are requested to arrive for registration at 9am for a 9.30am start.

Please note that attendance will be limited to 2 attendees per organisation.
An expression of interest to attend this Market Engagement Event is no indication of a commitment to participate in the future tender process nor does it infer any special status on those Providers who do submit an interest. Providers who do not attend this Event will still be able to apply for the Procurement in the future. Please note that this Event does not represent a formal stage in the procurement process and attendance will play no part in any subsequent evaluation process.
Any information provided by the CCG in the course of this Market Engagement Event would be made available to all interested parties responding to any future potential tendering opportunities.
