Tender Ref: T.LPT.041 Provision for the Supply and Delivery of Healthy Teeth Happy Smiles Oral Health Packs on behalf of Leicestershire Partnership NHS Trust - AWARD

Healthy Together is the name for the 0-19 Healthy Child Programme services (largely health visiting and school nursing services) which will continue to be provided by Leicestershire Partnership NHS Trust (LPT) under new contracting arrangements with Leicester City. LPT take up their responsibilities for delivering the programme on the 1st July 2017 when it will be known as Healthy Together Leicester City.

The scope of this contract is essential is for the preparation and delivery of healthy teeth smile packs. The contractor will be responsible for sourcing all pack contents apart from leaflets which will be provided by Leicester City Council.

Contract Duration for this tender is : one year + one year option (at LPT's discretion)

Scope of Supply

The scope of the contract is to supply and deliver the required number of Healthy Teeth Happy Smiles oral health packs as follows:

• oPacks A, B, C and E to be delivered monthly to 12 delivery addresses in the Leicester City area or to one central point in the City
• oPack D to be delivered to 83 schools once a year or to one central point in the Leicester City area
