description
The aim of this service is to provide quality care in their own home for patients who have entered the terminal phase of life, and for whom a clinical has determined they should receive care under the fast track provisions.
Fast track care will be provided so as to achieve the following overarching aims:
| • | To support the CCGs to enhance the quality of life for those in receipt of fast track care. |
| • | To deliver a high quality service ensuring service user dignity and respecting individual physical and cultural needs. |
| • | To work in partnership in the delivery of the service with other health care professionals, including (but not exclusively): |
| • | Local authority |
| • | Local acute NHS Trusts |
| • | Local community services providers |
| • | General Practitioner |
| • | Registered providers of care. |
| • | To achieve the performance standards set out in this specification |
| • | Hospices |
| • | Marie Curie |
The detailed objectives of the service are:
| • | The CCG fulfils its duties with regard to fast track care to commission high quality, safe and effective care that provides value for money. |
| • | The CCG complies with national operational guidance and standards with regard to NHS CHC (NHS England Operating Model for NHS Continuing Healthcare March 2015) |
| • | The CCG deals fairly and in a timely way with referrals for fast track care. |
| • | The CCG complies with all associated legislation including Mental Capacity and Deprivation of Liberty Safeguards |
| • | The CCG continually monitors and assures itself of the quality and safety of providers supplying fast track care in all associated care settings and, where quality or safety cannot be assured, acts to remedy this in a timely fashion and to inform other agencies of shortcomings, as appropriate. |
