AQP Individual Budgets for People with Long Term Conditions in Northamptonshire

Commissioners within NHS Nene and Corby CCG’s in partnership with Northamptonshire County Council wish to commission an AQP Individual Budget framework for adults who have significant and complex long term conditions including Mental Health, Learning Disabilities and Physical Health needs.

These Services will deliver a range of safe, high quality services across Northamptonshire that offer people choice of provider and type of intervention/ support. Support will be purchased by individuals who have been allocated individual budgets from Social Care for Mental Health needs or Health funded budgets for complex long term conditions. Services will need to meet a range of health needs including those with complex physical or mental health, learning disabilities, dual drugs and alcohol needs, individuals with forensic histories who may be subject to MoJ restrictions, individuals who are increasing in age and those may have additional physical needs. Services will need to be focussed on recovery, building on individuals strengths and be outcomes focussed creating an ethos of hope and opportunity. Services will be provided in various locations including individuals own homes and community locations. A range of providers is required including domiciliary support, day opportunities and services that can support with employability/ accessing employment.

The contract will be awarded for a period of 3 years commencing on the 1st April 2017.
