There are three services within this procurement. These are: the Expert Patient Programme, the Diabetes Champions and the Diabetes User Group.
The Expert Patient Programme is a generic six-week course for people with a Long Term Condition. The course is based upon the Chronic Disease Self-Management Programme developed by Stanford University. The course is licenced and the provider must purchase and meet the requirements of the licencing for the full duration of the contract. The Diabetes Champions is a Public Health initiative that recruits local people as volunteers, who represent the local population, to be trained as Diabetes Champions. The Diabetes Champions, with the support of a Diabetes Champion Coordinator, target key groups to raise awareness of the risk factors of Type 2 diabetes, encourage people to make healthier life choices and signpost people as is appropriate. The Diabetes Champions target people in a variety of ways and host events in addition to attending existing community groups and events.
The Diabetes User Group provides a targeted forum to ensure that the voices of the local population contribute directly to the provision and development of local diabetes services. The DUG will also play a key role in supporting the CCGs with developing its plans and strategies around local diabetes services and ensuring that the perspective and experience of local people with diabetes is at the heart of decision making.
All three services that form part of this procurement have specific outcomes, yet they all utilise the experience and passion of local people to improve the health of the broader population, specifically those with long term conditions. Providers should be able to identify how each service can be delivered to achieve the identified outcomes, but also the opportunities between managing all three services in order to achieve better outcomes overall.
Additional information: We would welcome attendance by smaller organisations working in the local area as this may provide a collaborative opportunity. The event will offer an opportunity to learn more about the elements of the service and the licencing requirements to deliver it.
