The Provision of a Cardiac Rehabilitation Service on behalf of NHS Heywood, Middleton and Rochdale Clinical Commissioning Group

NHS Heywood, Middleton and Rochdale Clinical Commissioning Group (CCG) are looking for suitably qualified Providers to submit tenders for The Provision of a Cardiac Rehabilitation Service.

The Provision of a Cardiac Rehabilitation Service purpose is to provide all patients who are 16 years and above who fulfill the referral criteria with a patient centred service which optimises their health and well-being, enhances quality of life and minimises the risk of recurrent cardiac events.

The objectives for this Service are detailed in the Service Specification.

The contract duration will be for 3 years with a Service Commencement date of 1st April 2017. The indicative annual value is £225,500. The contract will utilise a Local Full tariff payment adopted from the National Mandatory tariff and will be that of a stepped approach as detailed with the Tender Pack. The Services to which this Procurement relates fall within Schedule 3 of the Public Contracts Regulations 2015 and the tender process will be conducted as a single stage open process.
