Our joint ambition is to improve health outcomes for people in Merton. As part of this, we want to strengthen the role of community services in improving the health of the community and thereby reduce the need for acute care.
We want provision that is based on an integrated approach, with community services playing a leading role in enabling effective working between different professionals and organisations, so ensuring that people in Merton experience care that is seamless and well co-ordinated.
We know that there will be many changes over the years ahead. Hence, we are seeking a Provider who will work flexibly, responsively, creatively and in partnership with commissioners and other partners; who will foster a “can do” attitude amongst staff and embed a culture of continual learning and development across the organisation.
Putting people’s needs first and delivering improved outcomes for them must be at the core of all community service activities at all times.

We aim to ensure that:

We are seeking innovative services that embrace technology and new ways of working that facilitate the delivery of high quality, accessible services. We want Community Services that are:
| • | Outcomes focused; |
| • | • Easily accessible and locally focused; |
| • | • Integrated, well-coordinated and joined up; |
| • | • Rapidly responsive to needs and supportive of discharge in order to avoid unnecessary hospital admission; |
| • | • Enabling of continuous care with a seamless flow between planned and unplanned care; and |
| • | • Promoting of health and wellbeing. |
Potential bidders are invited to submit PQQs for each of the Lots:
Lot 1 – Community Health Services including:
| • | Proactive Planned Preventative Provision and Rapid Reactive Response; |
| • | Contraception and Sexual Health Service; |
| • | Healthy Child Services 0-5; |
| • | Healthy Child Services 5-19 (including giving school age immunisations); |
| • | Children’s Community Therapy and Specialist Healthcare Support and Co-ordination; and |
| • | Specialist Nursing for Children Looked After and Care Leavers and the Multi-agency Safeguarding Hub (MASH). |
Lot 2 - Adult Musculoskeletal and Outpatient Physiotherapy Services
At this stage in the procurement the Commissioner can confirm that the contract value will be in the region of:
| • | Lot 1 Community Health Services - £21m to £31m per annum |
| • | Lot 2 Adult Musculoskeletal Outpatient Physiotherapy Services - £0.8m to £1.5m per annum |
Full details regarding the financial envelope will be provided at ITT stage along with further detail regarding the remuneration model. At this stage the contract is intended to be a 3 year term with one option to extend for one or two years to be confirmed at the ITT stage.
The deadline for PQQ responses is 07/04/2015 at 12 noon.
