This notice refers to a direct award of an interim contract to provide a GP Integrated Extended Access and Weekend Visiting Service on behalf of NHS Hillingdon CCG. This interim contract award will ensure continuity of essential health services to the local population. The GP Integrated Extended Access and Weekend Visiting Services are two key elements of this model, with the third element a proactive Care Home service model currently in development. Following the successful pilots of the GP Integrated Extended Access and Weekend Visiting services the CCG has assessed that Hillingdon Primary Care Confederation is the only capable provider at this time taking into account an on-going development and the significantly challenging budget that they work within. Critically, this provider is also in a unique position to be able to access patient lists with the full support and approval of the Primary Care providers across the locality. An integrated 2 year contract will enable the Hillingdon Primary Care Confederation to ensure efficiency, flexibility and sustainability to meet the demands of the patients within Hillingdon as well as supporting core general practice through releasing pressure on surgeries to allow greater focus on proactive models of care. This interim service will support the CCG to review its commissioning intentions and the model of service to be established will ensure integrated pathways to deliver holistic patient care aligned with NWL strategies. This interim contract award is necessary to enable NHS Hillingdon CCG to develop and validate its specified requirements and commissioning approach for the long term future of the service.
