The WLCCG service model below illustrates the provision of urgent care across a twenty four hour period for both ambulatory and non-ambulatory patients. This model recognises the following:
| • | During the week day patients will present to their own registered practice with urgent care presentations and GPs will undertake home visits as part of their core service provision |
| • | Some patients will have urgent care clinical presentations during GP core hours but are too unwell to travel or are bed bound and require a home visiting service. This will be provided through the urgent care home visiting service |
| • | Some patients will have urgent care clinical presentations during GP core hours that require a range of diagnostics and clinical assessment that can be offered in a community setting. This will be provided through the Community Super Hub (currently Loughborough urgent care centre) |
| • | All patients with an urgent care need after their registered practice has closed can be offered assessment and treatment in a community setting or in their own home following appropriate triage through the clinical navigation hub. This was previously delivered through the LLR OOH service but in the new model ambulatory patients will receive urgent care through Tier 1 extended primary care and Tier 3 the Community Super Hub. Non-ambulatory patients will receive urgent care through the home visiting service. |
This service will be accessible and provided to all registered patients residing in the WL CCG area. The service is also available to unregistered patients but the provider is expected to make arrangements for the patient to register with a GP. This service is also available to patients registered outside of the CCG boundary.
It is expected that out of county patients can also access the service but there will be recharging to the appropriate organisation.
Additional information: There is the expectation that this bidder will join the anticipated Leicester, Leicestershire and Rutland (LLR) Urgent Care Alliance.
