Doncaster CCG would like to commission several pieces of work to enable hospital patients whose needs are hard to meet to be successfully discharged into the community as part of the Assuring Transformation process.
There is a small cohort of patients, currently detained under the Mental Health Act, with a diagnosis of Autistic Spectrum Disorder, often with co-morbid diagnoses, who require Applied Behavioural Analysis to identify their unmet needs and to develop and implement effective external boundaries and care techniques to reduce the unwanted behaviours that present a barrier to discharge.
An additional task will be to guide the commissioning of robust community provision for this cohort. It is envisaged that bespoke single person services will be required to meet the niche needs of each of these current hospital patients.
The Applied Behavioural Analysis must be carried out by a suitably qualified practitioner who can provide an assessment of current behaviours, a thorough assessment of historical information; can develop, implement, monitor and review effective intervention plans in situ and based on this, provide useful advice to guide the commissioning of longer term community based provision.
