description
The CCG & partners seek a suitably qualified organisation to complete a time limited piece of work to develop a plan for the delivery of Local Care Networks (LCNs). LCNs are a local delivery mechanism that will support integrated working and improved outcomes for the local population.
The development of a high level model for South East London has been agreed through the strategic programme "Our Healthier South East London". The LCN forms part of our Community Based Care model.
The key partners have funded a programme lead to support this work and have set up a Programme Board to support delivery. The work stream now requires additional consultancy support to drive forward improvements and delivery of the model.
