NHS West London Clinical Commissioning Group (WLCCG) is looking to engage with appropriately qualified and experienced providers to express their interest to inform the design of a robust model through OD and Rapid Learning for the CCG's integrated care offer for the 65 and over called My Care, My Way.
The aim of this early market engagement exercise is also to gauge potential interest in this contract opportunity and field for innovative proposals to improve the current drafted service specification (attached).
Feedback received will assist to determine the WLCCG procurement strategy including whether a competitive procurement process is appropriate.
The intention for the service is to provide Organisational Development to support the delivery of the full Model of Care in a number of exemplar teams and to work with multiple agencies to refine and test and refine again the Model of Care. This rapid learning process will ultimately lead to an enhanced Model of Care which can then be rolled out across the entire programme. It will enable commissioners to develop an alliance approach with providers to deliver the model of Care which can then be adopted to deliver a range of outcomes agreed across the alliance.
It is intended that this service will:
| • | create an open and honest culture which supports people to be courageous in delivering better health outcomes; |
| • | assist every individual member of the My Care My Way team and the wider community partners to understand their role and work towards a common purpose; |
| • | enable the organisational structure of the My Care My Way programme is clear and meets the highest standards of governance and enables WLCCG to achieve its strategic vision; |
| • | assist in the refinement of the Model of Care, developing partnership working with the knowledge, skills, behaviours and qualities required to deliver the transformational changes required to meet future needs. |
The expected 4 month contract value will be circa £160,000.
West London CCG welcomes interested parties from all market sectors, including the Voluntary Sector and the service will possibly commence during the summer of 2017, if not earlier.
