NHS Wandsworth CCG intends to establish a locally delivered Social Prescribing Service which will have a specific focus to support the improvement of social and clinical outcomes for people with long-term conditions and their carers, whilst optimising use of NHS and social care resources.
Taking learning and wishing to expand upon a similar service that is already in place in Merton, the Wandsworth service will be rolled out using a phased approach. The service will operate from within Primary Care practices across Wandsworth and will work closely with the Wandsworth Wellbeing Hub and the Merton Social Prescribing Service.
The contract is likely to be for a duration of 2 years with an option to extend this for up to a further 2 years (potentially 4 years in total), with the service being provided within a budget of around £200k per year (final value pending confirmation).

The CCG would like to invite interest in providing this new service across Wandsworth so that it can be informed ahead of a potential procurement process to establish a contract. The CCG is currently planning for a contract to be awarded by June 2019 so that mobilisation can commence to run the service from September 2019.
