The Borough Council of Bolton is prepared to receive tenders for Complex Lifestyles Service which will be subject to the information, specification and tender package enclosed. The term of the Contract will be for 1 year with the option to extend for a further 1 year. It is intended for the Contract to commence on 1 December 2015. The Authority is requesting a Provider to provide a Service that is responsive, coordinated, proactive, focused, and specific to each individual Client, to improve outcomes for adults in Bolton with a sub-set of complex needs. This sub-set is defined as individuals who repeatedly use emergency interventions presenting with 3 or more of the following: · Alcohol or drugs misuse · Mental ill health (including self-harm behaviours and risk of suicide) · Offending or risk of offending (or victim of crime) · Poor/unstable accommodation or homelessness
