description
The aim of the Making Connections Walsall (MCW) Programme is to tackle loneliness and social isolation of older people, improve wellbeing and reduce preventable Health Service and Social Care Service usage. It will take a Social Prescribing approach to improve the social connectedness of older people (including the vulnerable and marginalised) within the allocated locality areas and across Walsall.
This Service is for older people who are lonely and or Socially Isolated. The scope of this element of the project is to act as a single point of contact for the Making Connection Walsall Initiative.
As the independent referring organisation you will receive referrals from GPs, and/or Health & Social Care professionals or community and voluntary sector and self-referral. There will be one telephone number allocated to the Making Connections Walsall project which the Provider will supply.
The Provider is required to receive phone calls and input service user referrals onto the agreed Data collection system 24 hours per day 7 days per week.
The Provider must currently have the infrastructure in place to meet the needs of the MCW Programme.
For each individual referral the provider will ensure a Safe and Well visit is carried out if one has not already been undertaken.
