The Enhanced Physform pilot aims to improve the physical health of patients with serious mental illness (SMI) and address known health inequalities. Patients with an SMI are reported to have a reduced life expectancy of up to 25 years compared to the general population, an inequality that is mainly related to patients with cardiovascular, respiratory diseases, diabetes and increased risk of cancer and liver disease. The project will require GP Practices to carry out a face to face 6 month physical health review with patients who have an SMI, carry out brief interventions to improve lifestyle and discuss action planning to improve the patient’s health.

The aim of the evaluation is to independently evaluate the effectiveness of the Enhanced Physform Project against the three key project objectives

  • Improved knowledge of the needs of patients with SMI amongst healthcare professionals (via an education event and patient reviews)
  • Integrated approach to monitoring, managing and supporting patients with SMI (via GP brief intervention and action planning with patients and dialogue with appropriate physical/ mental health service teams)
  • Encourage patients with SMI to take active role in the management of their own physical health (via signposting and physical health action planning)

The project implementation and outcomes will be evaluated to establish to what extent project objectives have been achieved in the appropriate timeframe and identify any variance in outcomes or barriers to success. This will include a measure of improvements in specific health outcomes, GP Practice reported improvements, reflection on how to meet the needs of patients and patient feedback

The evaluation team will be required to participate from the beginning and attend the regular steering group meetings (first meeting scheduled for 5th November 2015), in order to understand the impact of each stage of the project. There will be scope for the evaluation team to influence the data and outcome tools required for completion by the GP Practice and view the project progress against the objectives. The steering group meetings and the education events will provide opportunity to capture current behaviours, understanding and potential barriers for patients and professionals.
