Provision of a Remote Care Monitoring Solution to deploy remote diagnostics across 8-10 care homes for a period of one year from the start of the contract. Across the usage period we wish to test the impact of the solution on the following:

| 2. | Improvements in GP ability to prevent call out of emergency services. |
| 3. | Reductions in unnecessary conveyance to hospital. |
| 4. | Reductions in emergency admissions. |
