NHS Nottingham City CCG seeks to appoint a suitably qualified provider to deliver high quality therapy led community based specialist rehabilitation for patients with any advanced or progressive illness, regardless of diagnosis identified with palliative or end of life care needs to achieve the following outcomes:

Patients experience increased independence following intervention.
Patients are maintained in their own home environment (where appropriate).
A reduction in unplanned hospital admissions.
A more co-ordinated approach between community and secondary care services.
Improved experience and choice to patients at end of life.

Applicants will be required to submit the completed tender documentation by 2nd September 2016. The anticipated contract start date for this service will be 1st December 2016.

Additional information: Arden and Gem CSU is managing this Procurement in accordance with the Public Contracts Regulations 2015 (the “Regulations”). The Service(s) is/are being procured as “Schedule 3 — Social and Other Specific Services” and AGCSU is bound only by those parts of the Regulations applying to Schedule 3 services.
