Prior Information Notice:

Ealing Clinical Commissioning Group wish to give prior notification of their intention to commission Community Based Care Navigation Service for patients registered with GPs in the borough. Care Navigation refers to assistance offered to patients and carers in ‘navigating’ through the complex health and social care systems to overcome barriers in accessing quality care and treatment and in particular accessing non-statutory services, working closely with the wider GP practice team and care coordinators.

Invitation to Prior Information Notice event: Thursday 7th July 2016 at 11:00 in London.

The Commissioner would like to invite interested potential providers to attend a Prior Information Event on Thursday 7th July at NHS Ealing Clinical Commissioning Group, 3rd Floor, Perceval House, 14/16 Uxbridge Road, Ealing, W5 2HL.

Interested parties are requested to register for this event by 12:00 on Monday 4th July 2016 by email to: ealccg.cnproject@nhs.net

The Event will provide opportunities to:

  • Understand the intention of the service;
  • Understand Commissioner's proposed future model of care;
  • Understand the forthcoming procurement process;
  • Meet and network with peers and/or potential partners.

To ensure that the future Care Navigation Service provides the best all round outcomes for service users, the CCG is interested to consider the views and advise of those with experience in the delivery of Care Navigation Services.

In all cases, the CCG alone will retain responsibility for deciding on the contents of all procurement documents.

If the CCG decides to proceed with procurement, this is likely to begin in the middle of July 2016

Additional information: General:

NHS Ealing Clinical Commissioning Group is intending to tender for a community based Care Navigation Service to work with patients with long term conditions to improve their quality of life and health outcomes. It is expected that bidders will work under a ‘lead provider’ model but the CCG is prepared to consider all operating models which effectively provide the serve.

The objectives of the service include:

+Improve outcomes for patients through the provision of care navigation;

+Work with the care coordination service to reduce non-elective admissions, improve social inclusion and reduce missed appointments; +Provide care closer to home in-line with the Out of Hospital and Whole Systems Integrated Care models.

Ealing CCG is responsible for the health of over 417 420 people registered with GPs in Acton, Ealing, Hanwell, Northolt, Perivale and Southall. The CCG plans and commissions care and treatment to meet local health needs. A small percentage of patients registered with Ealing GPs reside across the borders in neighbouring CCGs areas.

Ealing CCG commissions services from providers including community services, mental health services, acute, secondary and tertiary care. Primary healthcare services in Ealing comprise of 79 GP practices; 42 general dental practices; 54 community pharmacies and 41 optical providers.

Any expenditure, work or effort undertaken by organisations or individuals prior to any subsequent procurement for these services is a matter solely for their own commercial judgment.

The CCG reserves the right to withdraw from the process at any time or to not tender these services or to tender on a different basis.
