Expression of Interest for APMS GP Services for Kirkby Community Primary Care Centre and Balderton Primary Care Centre in Nottinghamshire

NHS Greater East Midlands Commissioning Support Unit (ARDEN & GEM CSU) on behalf of NHS England (North Midlands), Mansfield and Ashfield and Newark and Sherwood Clinical Commissioning Groups are inviting expressions of interest from suitably qualified providers interested in the re-procurement opportunity for the Commissioning of GP Services on an APMS contract. The procurement will be in the following lots:

Lot 1 - Kirkby Community Primary Care Centre – Kirkby in Ashfield, Nottinghamshire and Lot 2 - Balderton Primary Care Centre – Newark, Nottinghamshire .

NHS England and Commissioners are looking for a modern, patient-centred model that is user friendly and culturally sensitive that considers all of the national and local public health targets. The model will need to include provision of the following:

• Essential Services
• All Additional Services
• Directed Enhanced Services
• Development of innovative service provision with Clinical Commissioning Groups around strategic ambitions which include integration, collaboration and use of technologies

The Contracts will be for a period of 5 years with an option to extend for up 5 years (60 months).
