The Health and Social Care Information Centre (HSCIC) is tendering for a Supplier to design, disseminate and collate questionnaires and supporting documents for a survey of people with diabetes when they stay in hospital (as inpatients), across England and Wales and provide the subsequent data-entry/handling between the months of June 2016 to November 2016.
