Discharge to Assess (D2A) is designed to reduce unnecessarily prolonged acute hospital stay. It is a multi-agency approach between the Authority and SWFT including Continuing Health Care, which allows assessment for ongoing services to take place after discharge from hospital.
There are three pathways within D2A:
Pathway 1: For customers who can return home with additional support from CERT or Reablement.
Pathway 2: For customers who cannot be discharged directly home but who have the potential to do so if given additional rehabilitation.
Pathway 3: This is for customers who are likely to need a care or nursing home placement in the longer term
The scope of this tender is the commissioning of residential home places (beds) from which Pathway 2 services can be delivered.

SWFT is seeking up to 20 places across 2 homes in Warwick and Leamington Spa: Postcodes - CV31, CV32 and CV34
If we are unable to source the beds in just 2 homes then we will consider a third home.
If we are unable to source the beds in 3 homes within Warwick and Leamington then we will consider homes in Kenilworth Postcodes - CV8.
If we are unable to source the beds in Kenilworth then we will consider Stratford –upon-Avon: Postcodes - CV35, CV37, CV33 and CV47
Note: Only applicants who propose homes that are in the Postcodes listed above will have their submissions evaluated. Applicants whose homes are outside these areas should not apply as they will not be eligible for an award.
The contract is for the period of 12 months with an option to extend for further period not exceeding 6 months
