Arden & Greater East Midlands CSU, on behalf of Sandwell & West Birmingham CCG are looking to hold a market engagement in order to understand the market for offering a service to individuals considered to be High Intensity Users who have frequent unplanned hospital admissions. The service will work across health and social care to support service users to access services which can meet their needs in a timely and effective manner. This is anticipated to include:
- Effectively connect with, support and coordinate high intensity ambulance callers and A&E attenders within the SWBCCG footprint
- Reduce the activity High Intensity Users have in GP practices and non-elective admissions
- Establish, utilise and coordinate connections with multi-agency and existing professional services to negotiate a new and innovative way forward
- Demonstrate a reduced workload on unscheduled care services and the wider health economy resulting from reduced 999 calls, which otherwise would have attended A&E, result in an admission, or a call to the police
- Safely manage and coordinate the needs of the client group through the use of robust community and service connections and the voluntary sector
- Support patient choice and compliance
- Provide fertile commissioning intelligence across all providers and in doing so, lower the stigma associated with high intensity users
- Create and coordinate a replicable service which can be integrated and managed over the longer term across other providers.
