Integrated Community Cardiovascular Services

Newham CCG aims to select by formal tender a single main provider, using a prime contractor (provider) model of contracting and the EU negotiated procedure.

The prime contractor will be responsible for the provision of the whole cardiac pathway and will also be responsible for sub-contracting elements of the services to its partner providers.

The Prime Contractor shall be directly responsible for managing the total pathway including the clinical outcomes, key performance indicators and financial payments to its sub-contractors.

It is not envisaged that one provider alone will provide all the services required, as this would impact on the rights of patient Choice, and also the whole-system need for service management.

It is envisaged that the “One Stop” model of community-based cardiology will be redesigned.

Newham’s Joint Strategic Needs Assessment (JSNA) and commissioning intentions identify cardiac care as a high priority for redesign, building on the achievements of the current community cardiology service.

The Newham vision is for care to be more integrated and accessible, to promote earlier diagnosis and better care; to empower patients to self-manage; and to reduce the incidence of CVD and its associated complications.

The key elements of the new service will be:

A requirement for the Integrated Community Cardiology Service (ICCS) to be staffed by a Multidisciplinary Team consisting of consultant support and oversight , GPwSI, specialist cardiology nurses and Heart Failure Nurses.

It should be noted that Newham’s proposed service requires fundamental changes in pathways, including how patients are treated. It is not an outreach clinic from secondary care or another intermediate service. The key differentiators include:

Potential providers with relevant technical capability and experience to deliver this service are required to express an interest if they are interested in bidding for this service should the CCG decide to carry out a competitive tender exercise and publishes a Contract Notice for this requirement.

Bidders who have expressed an interest in this market testing exercise will have to submit a separate response to the Contract Notice should the CCG decide to procure this service and publish a Contract Notice (advert) for this requirement.
