The videos are about the future of integrated health and social care - linking up the NHS, mental health services and council social care to provide joined up care for local residents.
The four videos will be:
- Getting you out of hospital quicker
- Our independent living schemes
- Integrated health hubs and integrated care in the future
- Helping you to live independently at home
Each video should be no longer than four minutes long.
