description

NHS Hammersmith & Fulham Clinical Commissioning Group (CCG) is commissioning a high quality, consultant-led integrated community cardiology and respiratory service for the residents of the Hammersmith & Fulham CCG.

Aims of the Service

The service aims to deliver an accessible, efficient, cost-effective service managing the majority of diagnostics and outpatient activity within primary and community care settings. The service supports the Out of Hospital ambitions of the CCG for the development of integrated pathways of care across health and social care, transforming care pathways by shifting care from acute to community and primary care settings where appropriate, avoiding hospital admissions if appropriate, and improving early discharge.

Further detailed information on the service can be found in the service specification and Memorandum of Information documents.

Contract Details

The contract to provide the service will last for an initial period of three years commencing from 1st April 2016 with the option for a further two year extension.
