description

\
The primary objective of the service is to enable people living in the Hartlepool community to remain living at home for as long as possible and maintain a good quality of life which meets their identified outcomes. People who use the service should be able to achieve and maintain their maximum levels of independence and self-care allowing them to live more independently with dignity. \
\
The providers will be expected to show a commitment to partnership working, quality assurance and continuous improvement. Performance will be monitored through self-assessment and the departments monitoring process for domiciliary care providers. \
\

\
Utilisation within the areas has remained fairly static with average hours delivered in each area in the region in the last 12 months of around 1900 - 2100 per week. The number of users within each geographical area range between 400 - 540 over ther last year.\
\
The Council will allocate the geographical area to the successful providers.\
\
The successful providers will be required to accept all referrals from the Council for any service user within their contracted geographical area unless following assessment they are unable to meet the persons needs.
