NOTE: This notice was updated on 04 May 2016 for the following reason: The closing date for submission has been extended to 23rd June 2016 10am.This service requires the provision of a Living Well Hub which supports the referrals into a range of commissioned health improvement services including Making Every Contact Count (MECC) on behalf of Stoke-on-Trent City Council.
