description

The Stockport locality has an approximate population of 317,000 residents, who are registered with one of 36 Practices in the area. In the line with both NHS E&I's and CCG's goal of having a Digital Frist approach to Primary Care delivery, the CCG is looking to commission supporting technologies on behalf of all Practices, to allow those patients with Digital access to be able contact their GP via appropriate digital channels for the delivery of healthcare, at time and place that is more convenient for them, including:

interactive online triage, that will enable patients to have their initial contact with the Practice to seek advice on a medical condition, request items such as repeat prescriptions, fit to work notes and book appointments.
Enable the patient to have a clinical consultation via an easy to use secure video consultation platform.
Allow the Practice to engage with patients via SMS texting for items including appointment reminders and links relevant advice & guidance.
Allow patients to respond to online questionnaires from the Practice, enabling the Practice to get better of understanding the services required by their patients and how current services are performing.

It is envisaged that the above will support Practices in streamlining workflow within the surgery, which will allow them to meet potential unmet demand and give additional time to support patients without access to Digital facilities, who in the main are senior citizens, often with complex needs.
