This Dementia Advisor Service will be procured as a block contract for 3 years, with an option to extend the Contract by up to 3 further periods of 1 year.
The overall purpose of the service is to enable people with dementia to live independently and well for as long as possible, and specifically to:
| • | Enhance people with dementia and their carers' understanding of the condition |
| • | Be someone who they can approach for help and advice at any stage of the illness |
| • | Be a single identifiable point of contact with knowledge of and direct access to the whole range of local services available. |
| • | As a consequence, people with dementia and their carers should experience a reduction in the occurrence of crises that could be avoided and an increase in their understanding of the condition and strategies for managing it. |
This service will need to work very closely with health and social care professionals from primary, secondary and acute care and will be complementary to the other elements of the dementia care pathway.
