Arden & GEM CSU is conducting this procurement exercise on behalf of the three Leicester, Leicestershire & Rutland Clinical Commissioning Groups, to seek suitably qualified and experienced providers to deliver the Intermediate Care Reablement service.
The objectives of this project are to commission an LLR wide solution for Pathway 3; this involves the integration of the Discharge to Assess placement (D2A), Non Weight Bearing (Placement) and reablement (placement) pathways. This integration will involve consolidating the existing 90 plus locations into just 3; one in each CCG area. It will be set in specific reablement environments, with reablement skilled care staff and a team of dedicated therapy staff. This will prevent any further deconditioning post transfer and enable patients to quickly transition into being able to optimise their self-care and improve their well-being This integrated care home based reablement service will enable patients initially unable to go home following discharge from the Acute sector, to have all their eligibly assessments completed promptly when fully optimised and away from the acute care setting.
The requirement includes care for and support to patients with cognitive needs as well as general care needs with 24/7 reablement support to prevent further deconditioning and actively promote self-care. All of this supported by on-site 7 days a week therapy to ensure all patients are assessed on transfer and personal goals set. This is to be coordinated by regular multi-disciplinary team meetings held as appropriate to the patients’ needs but at least on a weekly basis.

This is a new service requirement but it is anticipated that the service provision resultant of this procurement will have contract agreement values estimated as below:
Lot 1 Care Home ELRCCG £1,780,036; Lot 2 Care Home WLCCG; £1,780,036; Lot 3 Care Home LCCCG £1,780,36; Lot 4 Therapies service to Lots 1, 2 and 3 £2,495,421.
It is the intention of the Commissioners to enter into a 3 year contract with a possible extension option of a further 2 x 12 month periods.. The estimated values above cover the three year period from service commencement 1st November 2016 until 31st October 2019, and are shown not including VAT.
This service will be commissioned via an open procurement process. There will be two ITT’s which will be live on 14th April 2016 on the Arden GEM CSU e-tendering procurement portal (Bravo). One will be for the care home bed based service required for lots 1, 2 and 3, another for the therapies service lot 4.
