description

A request for quotes process was used to seek providers to provide a Domiciliary Care Rapid Discharge Service (RDS) in Lot 3a. The providers approached were identified in a tender exercise conducted in November 2020. The service is a short-term reablement and domiciliary service offering timely discharge of individuals who may have an eligible Care Act need. These individuals may need support with discharge from a Hospital setting to their own home or may require support to prevent the need for a hospital admission, thereby supporting flow and effective management within the wider system. The service may be used whilst a long-term package of care is being sourced via brokerage, to bridge a package of care until the existing provider has capacity to restart the package following a hospital admission, to provide an assessment period to enable a full Care Act assessment or to prevent a hospital admission. All individuals discharged into this service will be followed up by a member of Hampshire County Council's Reablement or Social care team.

Individuals discharged into this service may be Covid-19 positive and there is an expectation that the service provider will accept these individuals into this service. The Service Provider is expected to manage this need appropriately and in line with NHSE guidelines. https://www.gov.uk/government/publications/coronavirus-covid-19-providing-homecare/coronavirus-covid-19-provision-of-home-care

As a short-term service, the Provider will be expected to accept all referrals and begin packages of care within 24 hours of referral. The Provider will be able to provide care for the Individual for a maximum of 14 days until long term provision or other appropriate services may be sourced.

This service is based on the principles of Strength Based practices and continuing enablement to maximise the health, wellbeing, and independence of individuals throughout Hampshire.

The Council's expectation is that the Provider will deliver a reablement and a strength based domiciliary model of service delivery that meet the needs of all individuals including those with Dementia and other long-term health conditions/requirement.
